Community Resources
Social Determinants of Health are a critical aspect of whole person care. The Community Resource Vaults below were created to assist Provider Organizations in linking members to Community Organizations to address the conditions of the Social Determinants of Health. Each of the ten counties AzCH serves has resource information for the following four areas: Community-Based Social Services, Self-Help Referral Connections, Substance Misuse Treatment Support Services and School and Family Support Services. This information is provided as part of the AHCCCS Targeted Investment Project.
