Fax Number Changes for Ambetter Medically Billed Provider
Administered Drug Prior Authorization Requests & Web Portal Use Reminder

Effective April 1, 2024, Ambetter from Arizona Complete Health will have a new fax number for medically billed, provider administered drugs prior authorization (PA) requests.

 When submitting a PA request for a medical drug/buy-and-bill/J-code, on or after April 1, 2024, please fax the request to 833-893-1460.

The preferred method forsubmitting PA requests for medically billed, provider administered drugs continues to be through our secure Provider Web Portal. Your PA request feeds directly into our system and allows us to receive and respond faster when submitted through the portal.

Visit the Arizona Complete Health website > For Providers > Pre-Auth Check for additional information on the PA process and access to our Pre-Auth Check Tool. We encourage providers to use the Pre-Auth Check Tool to verify PA requirements before submitting a PA request.
