Annual VFC Enrollment Reminder 2024

Date: 07/15/24

Medicaid 2024 Vaccines for Children (VFC) Re-enrollment Period Underway!

Dear Provider and Office Staff:

This  communication  is  intended for  providers  who  treat Medicaid members  18  years  of  age  and  under  and enrolled in the Arizona Vaccines for Children Program (VFC).

As a reminder, the VFC program requires providers contracted with Arizona Complete Health-Complete Care Plan (AzCH-CCP) and Care1st Health Plan of Arizona (Care1st) who treat Medicaid members 18 years and under to annually re-enroll.

You  can  find  the  2024  VFC  Annual  Re-enrollment  resources  and  electronic  re-enrollment  provider  guide  at  Arizona Department of Health Website.

2024 VFC re-enrollment began May 1st, 2024 and closes August 31st, 2024. If a provider fails to re-enroll in 2024, that provider may be inactivated from the VFC program for non-compliance and members 18 years must be removed from their panel. The provider may be permitted to return to the VFC program by submitting a new enrollment, no earlier than one (1) year after the Notice of Action.

If your 2023 Provider Agreement expires, you will not be able to order VFC vaccines until your 2024 re-enrollment is submitted and approved. Ensure your vaccine ordering is not disrupted. PLEASE Do not wait until the last minute to complete your online re-enrollment. Please begin your re-enrollment process now.

If you have questions regarding the re-enrollment process or the online forms, please contact the Bureau of Immunization Services (formally the Arizona Immunization Program Office) phone (602) 364-3642 or (602) 364-3899,  email  ArizonaVFC@azdhs.gov  or  ASIISHelpDesk@azdhs.gov  or  contact  your  Immunization  Program Specialist directly.
