Updated Instructions for Submission of SED Identification Requests
April 23, 2025
This communication provides an update to the Serious Emotional Disturbance (SED) Determination Process Change communication that was distributed 4/3/2025.
Effective immediately, all SED Identification requests must be submitted via the Serious Emotional Disturbance (SED) Identification/Removal of Identification Request Form located at https://forms.office.com/r/K2Uq47njaA. The form is secure and approved for submitting Protected Health Information (PHI). A separate secure email sent to systemofcare@azahcccs.gov is no longer accepted.
Required information includes:
1. AHCCCS ID
2. FIRST AND LAST NAME OF CHILD OR ADOLESCENT
3. DOB
4. QUALIFYING DIAGNOSIS
5. CALOCUS SCORE (or 99)
6. DATE OF IDENTIFICATION
7. MEMBER’s CURRENT HEALTH PLAN and ENROLLMENT DATE [NEW]
A member is required to have an AHCCCS ID and open enrollment plan for AHCCCS to input information into the AHCCCS PMMIS system. In instances where an AHCCCS ID is not found and/or an enrollment is not active, providers shall coordinate with the ACC-RBHA/TRBHA Contractor (if applicable for Non-Title XIX/XXI eligibility) to ensure member eligibility records are created and updated. Providers shall coordinate with the identified ACC-RBHA/TRBHA Contractor to obtain the AHCCCS ID number and open enrollment, and then provide the information to AHCCCS so the SED identification process can be completed.
If any of the above information is missing, the child or adolescent will not be identified as SED.
