Medicaid 2025 Vaccines for Children (VFC) Re-enrollment Period

This communication is intended for primary care providers (PCPs) who are enrolled in the Arizona Vaccines for Children Program (VFC) and treat Medicaid members 18 years of age and under.

As a reminder, all PCPs serving members 18 years and younger are required to be registered as VFC providers and must use VFC vaccines. PCPs must re-enroll in the VFC program annually.

You can find the 2025 VFC Annual Re-enrollment resources and electronic re-enrollment provider guide at https://asiis.azdhs.gov/.

The 2025 VFC re-enrollment period is May 1st, 2025-August 31st, 2025. If you fail to re-enroll in 2025, you may be inactivated from the VFC program for non-compliance and members 18 years of age and under removed from your panel. You may be permitted to return to the VFC program by submitting a new enrollment, no earlier than one (1) year after the Notice of Action.

If your 2024 Provider Agreement expires, you will not be able to order VFC vaccines until your 2025 re-enrollment is submitted and approved. Please avoid any disruption to your practice’s VFC process by completing re-enrollment at your earliest opportunity.
