AzCH 835 File Update

Dear Arizona Complete Health (AzCH) Providers and Billing Staff:

THANK YOU for the feedback many of you provided us regarding challenges with our 835 files. We shared your feedback with our corporate team and have been working closely with them to make sure improvements are made.

Previous 835 Challenges

  1. When there was a forwarding negative (credit balance) the write-off(wo) indicator was used instead of the forwarding balance (fb) indicator creating confusion
  2. Lack of sufficient information to identify the specific patient accounts to which offsets should be applied

While we validated there are no errors within the claims payment system (the system is processing the recovery correctly), we understand these issues made it very difficult for you to reconcile accounts receivable when there was a forwarding balance followed by an offset on a subsequent 835.  

Corrections/Improvements to the 835

The following corrections and improvements were made to 835 files for claims processed after 10/23/20:

  1. Accurate reflection of the write-off (wo) and forwarding balance (fb) indicators
  2. The Patient Control Number and check tracker number were added to the forwarding balance segment (fb) of the 835 to assist in identifying the specific patient accounts to which an offset should be applied

Although at this time we are not able to recreate historical 835s, we can work with you to provide additional reporting to assist with balancing your account receivables when needed.
