Arizona Association of Health Plans (AzAHP) New Provider Roster Template Effective August 1, 2022

Dear Arizona Complete Health (AzCH) Providers and Credentialing Staff,
The Arizona Association of Health Plans (AzAHP) Credentialing Alliance has developed and approved a provider roster template for use by all non-delegated groups, i.e., groups for which the health plan is responsible for credentialing, for bulk practitioner adds, terms and changes.
The AzAHP Credentialing Alliance is requesting that this roster template be adopted as of August 1, 2022. Rosters submitted on alternate templates will be returned. Please Note: The AzAHP Practitioner Data Form is also available to submit individual practitioner additions.
The AzAHP Non Delegated Group Roster is available in the following locations:
