Community Consensus Collaborative (C3) Provider Survey

Arizona Complete Health-Complete Care Plan invites providers in the Southern GSA to participate in the Community Consensus Collaborative (C3) Survey. The AzCH-CCP C3 initiative is about bringing together ideas from across the community and from all walks of life.  This will help improve the health care system and address major challenges facing people with mental illness and/or substance use disorders.  Our C3 focus for 2023 will be on stigma and value-based purchasing.
