The Arizona Department of Health Services (ADHS) recently announced the application details for the $1m Collaborative Care Uptake Fund, which is designed to promote the integration of behavioral health services in primary care through the awarding of financial grants and technical assistance support. Here are some brief details:

RFGA2024-008 Collaborative Care Primary Care Physicians

Eligible medical groups must have

• No more than 50 employees
• Primary care physicians that practice in Arizona
• Intend to integrate behavioral health services into practice through the Collaborative Care Model (CoCM; CPT 99492-94  

Application deadline

• Wednesday, December 27, 2023 at 1pm MST
