Caregiver
Resources

Helping to care for a loved one is a rewarding experience. Sometimes it can also be a little challenging. Arizona Complete Health-Complete Care Plan believes that part of helping our members is also supporting those that care for them. One of the ways we do this is through offering the Stress-Busting Program for Family Caregivers. Learn more below!

Caregiver/Family Support Liaisons can help with:

  • Providing referrals and information on community resources in your area that support Caregivers. Examples may include support groups, educational events, and other networking and support opportunities. You are not alone.
  • Providing information on Arizona Complete Health-Long Term Care support programs, such as Stressbusting for Caregivers (more info below) that might help you as you care for your loved one.
  • Help explain the services and supports that your loved one is getting. We understand healthcare can be hard to understand, the Caregiver Liaison can help walk you through it.

  • Your Caregiver Liaison can participate with you and your loved one in Person-Centered Planning meetings, if you both choose, to provide additional support to you as needed.
  • Help get you access to the Member Portal
  • Make sure you know who to call if you have questions or concerns about your loved one and their care.
