Comprehensive Well Child Exam Reminder
This communication is intended for primary care physicians (PCPs) who treat Medicaid members up to 21 years of age.
The AHCCCS EPSDT program requires PCPs contracted with Arizona Complete Health-Complete Care Plan (AzCH-CCP) to provide a comprehensive well-child exam to members up to 21 years of age. PCPs must complete the AHCCCS’s EPSDT Clinical Sample Template** after each well-child visit and submit it to AzCH-CCP for review to ensure:
- The most current EPSDT Periodicity Schedule* is used at every EPSDT visit, and that
- All age-appropriate screenings and services are conducted during each visit.
To obtain a list of members in need of a well-child exam, please follow these instructions:
- Log into our secure provider portal
- Click on the Provider Analytics option on the right.
- Choose the Quality link under P4P and Quality Reporting
- Click on the blue button in the upper right-hand corner labeled ‘Gaps Member Detail’
- Select the small drop-down option in the middle of the screen
- Choose the applicable year and line of Business in the top highlighted section
- Select Click Drop Down Menu to Export to export a Member Detail Report to PDF or Excel
The Gap reports identify members assigned to your practice that may not show as established patients in your system and may be due for a well-child exam.

Remember to log your immunizations into the Arizona State Immunization Information System (ASIIS).
