Arizona’s Closed-Loop Referral Lunch & Learn Invitation
Arizona Complete Health-Complete Care Plan, in partnership with CommunityCares*, is excited to announce our Closed-Loop Referral Lunch & Learn series designed to support and empower healthcare providers and community-based organizations to effectively implement and utilize its system for screening and referring members for health-related social needs, ensuring services are delivered and outcomes are tracked. These lunch & learn sessions will provide valuable tools and resources to enhance your skills and knowledge.
The CommunityCares’ network, powered by the Unite Us platform, creates a common infrastructure for connecting health, human, and social service organizations in Arizona. Participation in CommunityCares is *free for Arizona organizations, and there are a variety of benefits for joining. The first step is to attend a lunch & learn session to learn about the network, partnerships, and see a demo of the platform.
Closed-Loop Referral Lunch & Learn Cadence: 2nd Tuesday of month

• Screening and Referral: Learning how to identify and screen for health-related social needs.

• Data and Reporting: Learning how to collect, analyze, and report on referral outcomes.
• System Integration: Integrating the closed-loop system with existing electronic health records (EHRs)
