AzAHP Credentialing Alliance Roster & Credentialing Form Updates

 

Dear Providers and Credentialing Staff:                                                                                       

Following feedback from our delegated groups and based on the recommendations and approval from the Arizona Alliance of Health Plans (AzAHP) Credentialing Alliance, we’ve made updates to rosters and credentialing forms:

• The AzAHP Rosters for both Delegated and Non-Delegated groups have been updated to make several fields optional
• Minor updates have been made to practitioner and facility credentialing forms

Please begin using the updated forms no later than October 1, 2025.
