Provider Requirements for Assisting Individuals with TITLE XIX/XXI Screening/Application Process
Behavioral health providers are required to assist individuals with applying for Arizona Public Programs (AHCCCS AMPM Policy 650), including Title XIX/XXI, Medicare Savings Programs, Nutrition Assistance, Medicare and Cash Assistance.
For individuals who are not identified as Title XIX/XXI eligible, providers shall assist individuals with the AHCCCS screening/application process for Title XIX/XXI or other Public Program eligibility through HEAPlus at the following times:
a. Upon initial request for behavioral health services,
b. At least annually, if still receiving behavioral health services, and
c. When significant changes occur in the individual’s financial status.
Additionally, Community Assistor Programs support Arizonans with applying for and renewing benefits online. Trained application assistors complete Health‑e‑Arizona Plus applications through an interactive interview process and can track application status while providing ongoing support throughout the eligibility and enrollment process.

Process overview: Screen eligibility → Assist with HEAPlus application (if needed) → Optional referral to Community Assistor Program → Follow up and document outcome.
Behavioral health providers must help individuals apply for Arizona Public Programs (AHCCCS AMPM Policy 650), including Title XIX/XXI, Medicare Savings Programs, Nutrition Assistance, Medicare, and Cash Assistance. In this process, “Title XIX/XXI eligible” means eligible for/enrolled in Medicaid (Title XIX) and/or CHIP (Title XXI), and “identified as eligible” means eligibility/enrollment has been confirmed using documentation or verification steps allowed in the provider workflow.
If an individual is not identified as Title XIX/XXI eligible, providers shall: complete an AHCCCS screening, assist with submitting an application for Title XIX/XXI or other Public Program eligibility through HEAPlus, and document the status and outcome. Complete these steps at the following times:
a. At the initial request for behavioral health services: screen and, when indicated, assist with a HEAPlus submission.
b. At least annually while the individual is receiving services: re-screen and update or renew through HEAPlus as needed.
c. When there is a significant change in financial status: re-screen and assist with any required HEAPlus application updates.
Additionally, Community Assistor Programs help Arizonans apply for and renew benefits online. Trained assistors can complete HEAPlus applications through an interactive interview, track application status, and provide ongoing support during eligibility and enrollment.
