Important Arizona's Children Association (AZCA) Update

Effective October 1, 2022, AZCA will close their Apache Junction, Sierra Vista, and Surprise locations.

AZCA will continue to operate their locations in:

1.      Buckeye/Goodyear

2.      Casa Grande

3.      Chandler

4.      Florence

5.      Gilbert

6.      Lake Havasu City

7.      Maryvale

8.      Nogales

9.      Phoenix

10.  Tucson

11.  Yuma

However, only Phoenix, Tucson, Nogales, Yuma, and Lake Havasu City are accepting referrals; the other sites remain on hold for intakes until further notice.

For members in the areas where AZCA sites are closing, referrals can be made to the other Health Homes available in those areas:

·         Cochise County: Community Bridges, Community Health Associates, Community Partners Inc., Easter Seals Blake Foundation, Pinal Hispanic Council, and La Frontera-SEABHS

·         Maricopa County: Community Bridges, Horizon Health and Wellness, and La Frontera-Empact

·         Pinal County: Community Health Associates, Easter Seals Blake Foundation, La Frontera-Empact, Horizon Health and Wellness, Intermountain Centers for Human Development, and Pinal Hispanic Council
