Spectrum The Net Program Announcement

Arizona Complete Health, in partnership with Spectrum Health Group, is pleased to announce a new program to address the growing number of members who lose contact with services after leaving out-of-home (OOH) facilities (i.e. IP, BHRF, etc.). Spectrum Health Group’s, The Net, program will meet members wherever they are in their healthcare journey.

The Net extends peer support services to members transitioning from higher levels of care. Experience and data have demonstrated that when members leave OOH placements without being connected to outpatient services, they are more likely to return (i.e. readmit) to higher levels of care. The Net will bridge this gap by working closely with members to provide intensive care coordination and by addressing social determinants of health. Through this wrap-around approach, The Net will optimize members’ health outcomes and prevent readmissions. The Net will keep members connected to their existing outpatient team(s) and/or help them establish with new outpatient providers.

The Net is now live in Maricopa County!
