How Can a Member Sign Up for Mail Order?

Members can register for our preferred Mail Order Service one of the following ways: 

  1. Phone:  Express Scripts® Pharmacy Member Services at 1-833-750-0201 (TTY: 711) 24 hours a day, 7 days a week. 
  2. On-line: express-scripts.com/rx.  
  3. Mail: Complete the Express Scripts® Pharmacy form below and send it to theaddress listed on the form.

Once registered, members can use the Express Scripts® Pharmacy app to managemedications on the go.

  • Refill prescriptions, track orders, check medication prices, even make paymentsanytime, anywhere.
