DMG CRS Closure

District Medical Group Children’s Rehabilitative Services Clinic Closure

District Medical Group (DMG) will be closing their Children’s Rehabilitative Services (CRS) Clinic located at 3141 N. 3rd Ave., #100, Phoenix, AZ 85013 on Friday, July 18th, 2025. The last day the clinic will see members will be Friday, June 27th, 2025, however the clinic will remain open until Friday, July 18, 2025 to assist with the transition and care coordination activities. This closure will not change the services that AHCCCS members are eligible to receive, but members receiving services at this clinic will need to change where they receive their services.

The DMG CRS Clinic was the Multi-Specialty Interdisciplinary Clinic (MSIC) in the Phoenix area. MSICs provide a “one-stop shop” for primary care, diagnostic, dental, and specialty services in a single location, including high-touch care coordination, case management, and multidisciplinary/interdisciplinary teams. The clinic specialized in serving members with complex medical conditions, including those with a CRS qualifying diagnosis.

There are three additional MSICs in Arizona:

·        Flagstaff – Children’s Health Center NAH

·        Tucson – Children’s Clinic

·        Yuma – Children’s Rehabilitative Services

DMG CRS is committed to providing medical care through June 27th, 2025 and will work with families during this time to ensure continuity and continuation of care. DMG CRS is encouraging families to begin the process of transferring care to another appropriate provider or care setting within their  health plan’s network. Medical records will remain with DMG CRS and families can contact DMG CRS or access the DMG CRS patient portal to have records transferred to a new provider.

DMG CRS is sending out communications regarding this change to all members who have been seen within the past five years.

AHCCCS is working closely with DMG CRS and Arizona Complete Health (AzCH) to facilitate the transition of member care to other provider locations. AzCH and the other AHCCCS health plans are required to track members transitioning to alternative providers through the member’s first visit to the new provider for each service received by the member.
