Effective 10/9/2025 Wellcare is partnering with Conversio Health to offer an innovative program designed to improve medication adherence, health outcomes, and patient satisfaction for Medicare members with Chronic Obstructive Pulmonary Disease (COPD).

Conversio Health Services Include:

• Chronic Respiratory Management Program to support high-risk COPD patients
•  Home delivery of inhalers, nebulizer medications, and smart devices (no member copay)
• Virtual Pulmonary Rehabilitation including exercises, breathing techniques, health education, and smoking cessation

• 50%+ reduction in pharmacy costs
• 30%+ reduction in ER visits and hospital admissions

Cost to Members:

There is no co-pay,
coinsurance, or deductible for nebulizer-based medications, devices and
services provided by Conversio Health. Standard co-pays may apply for other
medications, (e.g., inhalers), depending on the drug tier and plan.  

Enrollment Options:

Conversio may contact you
directly to enroll eligible patients. You can also refer high-risk Medicare
COPD patients using one of the following methods:

• E-prescription: Add Conversio products as custom items to your EHR’s drug/item list. Contact Conversio for their e-prescribing instructions.
