FORMULARY UPDATES for Blood Glucose Testing Products
Effective 11/01/2025
Effective November 1, 2025, Arizona Complete Health-Complete Care Plan (AzCH-CCP) will change its preferred test strip brand from OneTouch to Accu-Chek.
Pharmacies can use the following information to process a claim for one Accu-Chek Guide Me Meter at no charge:
