Following the passage of AZ Senate Bill 1741 in June 2025, AHCCCS expanded healthcare coverage for adults aged 21 and older. Effective October 1, 2025, outpatient speech therapy and cochlear implants, previously limited to children, are now covered benefits for eligible adults.
What’s New
- Speech Therapy is now covered in outpatient settings for adults aged 21 and older
- Cochlear Implants are available for adults aged 21 and older following medical evaluation demonstrating medical necessity
Arizona Complete Health-Complete Care Plan (AzCH-CCP) will require prior authorization (PA) for specific cochlear implant services for members 21 years and older.
The table below outlines the cochlear implants that require PA effective November 10, 2025:

Note: Cochlear Implants already require PA for members under 21 years of age.

- Non-par providers and facilities must obtain PA for ALL SERVICES unless the Pre-Auth Check Tool states otherwise
- Verify eligibility and benefits before providing services. PA approval does not guarantee payment
- Use the Pre-Auth Check Tool at www.arizonacompletehealth.com -> For Providers -> Pre-Auth Check to confirm if a CPT/HCPCS code requires PA
PA Submission Timeline
PA requests for cochlear services may be submitted starting October 10, 2025, for services scheduled on or after November 10, 2025.
