Tenderheart Program for Pregnant & New Parent Members

Arizona Complete Health-Complete Care Plan is pleased to inform you that effective October 1, 2025, we will offer a new value-added benefit to support our pregnant members.

Eligible members may receive one of the following items at no cost:

• A Car Seat
• A Stroller
• A Pack-N-Play

These rewards will be provided by our trusted vendor, Tenderheart.

To qualify, members must:

1. Complete a prenatal care visit within their first trimester, and
2. Submit a Notification of Pregnancy (NOP) form to the health plan.

The NOP form can be submitted by the member through the member portal: https://www.azcompletehealth.com/members/medicaid/resources/family-and-community/start-smart-for-your-baby.html

Once both the first trimester prenatal visit and the NOP form are submitted, you may notify the member they can request their reward by visiting https://referrals.tenderheart.com/PregnancyRewards/ACH
