EPSDT Provider Reminder

Date: 12/08/21

This communication is intended for primary care physicians who treat Medicaid members under 21 years of age.

As a reminder, the EPSDT program requires primary care physicians contracted with Arizona Complete Health-Complete Care Plan to provide a comprehensive well child exam to members under 21 years of age per the AHCCCS EPSDT Periodicity Schedule.

According to our records, you may have a member(s) assigned to your panel that is overdue for their annual well child exam according to the AHCCCS Early Periodic Screening and Diagnostic Testing (EPSDT) Periodicity Schedule.  The Periodicity Schedule is linked below:

https://www.azahcccs.gov/shared/Downloads/MedicalPolicyManual/400/430_AttachmentA.docx

To obtain a list of members in need of a well-child exam, please log into your secure Provider Portal account at https://www.azcompletehealth.com/providers/login.html . Click on the Provider Analytics option on the right. Next, choose the Quality link under P4P and Quality Reporting then click on the blue button in the upper right-hand corner labeled ‘Gaps Member Detail’.  In the middle of the screen there is a small drop-down option that allows you to export your Member Detail Report to PDF or Excel.

As always, please verify whether a member is due for well child exam and/or immunizations when scheduling appointments. Members auto assigned to your practice may be due or overdue for a well-child exam and may not show as established patients in your system.

You can fax or mail your completed EPSDT tracking forms to:
