CHA Site Closures Effective March 2023

Effective March 1, 2023, Community Health Associates (CHA) will close their sites located in Casa Grande, Green Valley, and Nogales. CHA will continue to operate their locations in:

1.      Bisbee

2.      Douglas

3.      Parker

4.      San Luis

5.      Tucson

6.      Yuma

AzCH-CCP is working closely with CHA to ensure that current members are appropriately transferred. For members in the areas where CHA sites are closing, new referrals can be made to the other Health Homes available in those areas:

·       Pima County: COPE Community Services and Spectrum Healthcare Group

·       Pinal County: Easter Seals Blake Foundation, La Frontera-Empact, Horizon Health and Wellness, Intermountain Centers for Human Development, Pinal Hispanic Council and Spectrum Healthcare Group

·       Santa Cruz County: Arizona’s Children Association, Intermountain Centers for Human Development, Pinal Hispanic Council, and Spectrum Healthcare Group
